Guideline for reporting an Admin Abuser

The admin's steamid (You can get his steamid by typing status in the console.)
Admin's CS name.
Guideline for a Ban Request
Post Title: Ban Request: <Name of the person that you want to be banned.>
Name of the client that you want to be banned
Server IP
Client's Steam ID, (Get it by typing "status" in the console.)
Reason
Please attach a demo or show us some kind of proof how the person is causing problems in the server
